Procedures for Requesting Accommodations
In compliance with the Americans with Disabilities Act and Section 504 of the Rehabilitation Act of 1973 and Title IX of the Educational Amendments Act of 1972, students must voluntarily disclose their disability or pregnancy to the Student Counselor and request reasonable accommodations following the procedures below:
- The student must provide current documentation of the disability or pregnancy from a physician, psychologist, or other appropriate professional clearly stating the diagnosis and recommendations for needed accommodations. It should include how the diagnosis was made, and it must be signed by a professional in a medical or mental health field who is licensed/certified in the area for which the diagnosis is made.
- The student meets in person or by phone with the Student Counselor to discuss the limitations and current impact of the disability or medical condition. A Voluntary Self-Disclosure form, available in the Student Counselor’s office or by email, is completed by the student. Accommodations should be requested at least two weeks prior to the start of the term or activity. Accommodations requested after that will be provided as soon as possible.
- The Student Counselor completes an Accommodations for the Classroom form to document the accommodations that the college will provide as determined from the student and counselor meeting and the appropriate medical or mental health professional documentation. The Student Counselor emails the Classroom Accommodations form to instructors for online classes and provides a copy to be delivered by the student for on-campus classes. Students must always discuss accommodations with their instructors.
- At the beginning of each semester, the student is responsible for notifying the Student Counselor that accommodations will be needed for the semester. The student must inform the Student Counselor when there are any course changes so the appropriate instructors may be notified of accommodations.

All students, regardless of disability or pregnancy, must meet the essential technical standards of their program with or without reasonable accommodations. Details about attendance requirements and the appeals process are available in the SCC Catalog and Student Handbook. Students who wish to appeal an accommodation decision must submit a written request to the Dean of Student Services within five (5) business days of receiving or being denied accommodations.
